How is it some companies offer HD for channels and other companies don't?

Carriage agreements and bandwidth issues.
Channel X has reached an agreement with the providers that are carrying it, but hasn't the ones that aren't. Which in most cases the channel is asking for too much money, or the provider doesn't have the bandwidth to carry it.
